Currently hiring several Field Based Nurse Practitioners to provide in home assessments, chronic disease management and help bridge communication gaps by identifying chronic conditions within the Rochester, NY area.

Nurse Practitioner (NP)

Schedule: No Weekends!

• Full time: Monday-Friday, 40 hours per week
• Flexible start/end time
• No Weekends!
• Caseload: Typically, 5-7 patient visits per day (in person or telephonic)
• On-call only ~1 time per year - strictly telephonic - no home visits for on call!
• Flexibility in your schedule with autonomy while also having a supportive team environment. 

Compensation: $115,000 - $140,000/yr. base salary, plus a 10% performance bonus!

Nurse Practitioner (NP) responsibilities:

• Perform comprehensive annual and physical evaluations, chronic disease management for the Adult Geriatric patient population within patient home.
• Develop individualized care plans and ensure appropriate follow up through the care continuum
• Work collaboratively with an interdisciplinary team consisting of Physicians, RN’s, Dieticians, Pharmacists, Psychiatrists, Social Workers, Cardiologists and Podiatrists.
• Prescribe all necessary medications/treatments and educate patients/families regarding medication usage.

Qualifications:

• Desire to work with the Geriatric Adult Patient Population.
• Valid Nurse Practitioner (NP) license in the state of Pennsylvania.
• 1-2 years of Urgent Care, Primary Care or Internal Medicine required.
• BLS and ACLS Certification required. 
• NPI# as an NP
• DEA License.
• Valid driver’s license and willing to travel to patient homes/patient locations (facilities) and currently hold an active auto insurance policy.
• Minimum Master’s Degree in Nursing.
• Licensed Nurse Practitioner in the state of Pennsylvania.
• Expert in diagnosis and treatment of chronic and urgent medical/psychological conditions of the geriatric patient population.
• Experience in palliative care, and end of life communication.
• Home care experience preferred but not required.
