Job Overview

The nurse practitioner functions as an independent care provider in collaboration with other healthcare professionals. The care is characterized by comprehensive health assessments and management of patients with acute and/or chronic health problems in the heart and vascular service line for RWJBarnabas Health and Bergen New Bridge Medical Center.

Qualification

• Licensed Registered Nurse in the state of New Jersey
• Certified Nurse Practitioner in the state of New Jersey
• Masters/Doctorate in Nursing from an accredited program
• ACLS/BLS/DEA/CDS certification
• Must have cardiac and/or critical care experience
• Excellent communication and interpersonal skills
  
  

Scheduling Requirement

• 40 Hours per week
  
  

Essential Functions

• Conducts thorough patient assessments, including medical history and physical examinations and review of diagnostic test results.
• Develops and implements comprehensive care plans for patients undergoing invasive and non-invasive heart and vascular procedures.
• Monitor, review and manage all cardiovascular patients.
• Provides pre and post procedure care management for invasive and non-invasive cardiovascular patients.
• Cardiovascular education and support to patients and their families.
• Facilitates discharges for Heart and Vascular service line patients.
• Assist with in hospital Heart and Vascular service line patients evaluation, testing, coordinating and scheduling.
• Participates in quality improvement initiatives to enhance patient care and outcomes. Collaboration with a multidisciplinary team, including cardiologists, NP s, nurses and social workers.
