Position Description

Job Title: IHA Per Diem Nurse Practitioner/Physician Assistant

Reports To: Manager of Care Delivery

FLSA Status: Exempt

Location: Binghamton, NY (Candidates must be willing to travel within a 50-mile radius)

Schedule: Flexible; Days and Evenings (60–75-minute appointment times)

Position Summary

The Nurse Practitioner/Physician Assistant will conduct in-person, no-cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members. These assessments occur within members’ personal living spaces in accordance with market health plan expectations.

Key Responsibilities

  • Conduct 30 or more assessments per month within the designated county.
  • Deliver high-quality assessments that align with member satisfaction, health plan expectations, and compliance standards.
  • Submit chart notes within 24 hours of each visit and address queries within 48 hours of receipt.
  • Communicate effectively with clinical leadership and attend periodic training sessions.
  • Travel within a 50-mile radius as required.
  • Must be comfortable entering personal living spaces for scheduled assessments.
  • Ability to lift 20 lbs and bend or kneel for extended periods, as needed.

Qualifications

Education & Experience:

  • Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP).
  • Master’s degree from an accredited Physician Assistant program.
  • Home health experience required within the last five years.
  • In-home assessment experience preferred (minimum one year).

Competencies

  • Strong computer skills and proficiency with electronic medical records.
  • Familiarity with CMS risk adjustment models and the Affordable Care Act (ACA) guidelines.
  • Ability to accurately document assessment findings and maintain patient confidentiality.
  • Strong verbal and written communication skills to effectively interact with members and healthcare teams.
  • Ability to make critical decisions within policy guidelines and adapt to dynamic work responsibilities.

Certifications & Licensure

  • Current RN and NP or PA licensure in the practicing state.
  • Board certification from ANCC, AANP, or NCCPA.
  • Medicaid ID number for the state of practice.
  • Medicare PTAN Number.
  • BLS certification (active and valid).
  • Valid state driver’s license and proof of sufficient car insurance.

Compensation & Benefits

  • Compensation at market rate per completed assessment.
  • Mileage reimbursement for driven miles exceeding 30 per day, aligned with current IRS rates.
