Job Description

PURPOSE OF POSITION

Summary Of Duties And Responsibilities

The nurse practitioner (NP) is an advanced practice nurse responsible for comprehensive direct patient care management of the adult. As an independent clinical provider and utilizing the medical team as a resource, the NP provides expert clinical care to a designated population of patients providing services but not limited to:

  • Conducting history, psychosocial evaluations and physical examinations.
  • Analyze lab results/follow-up abnormal results.
  • Assessment and treatment of acute illnesses.
  • Follow-up of chronic illnesses.
  • As a PCP, has a defined panel of patients.
  • Administers First-Aid when necessary and is part of a facility emergency response team.
  • Participate in the Medical Board/Department Committee and meetings.
  • Formulate a plan of care management for the patient.
  • Participate in the different training/In-services conducted by the department or the facility/network.
  • Participate in the different QA issues in the department/facility (i.e.: Peer Review, etc.)
  • Complete patient correspondence (i.e.: forms or letters upon request).
  • Fosters good Customer Service in daily activities when dealing with patients, parents, (significant others).
  • Maintains Patient Safety and Infection Control techniques.
  • Follows corporate & hospital regulations, policies, & procedures.
  • Addresses patients, visitors and co-workers in a courteous and respectful manner.
  • Maintains a professional working relationship with other health care professionals.
  • Operates equipment and performs job related duties in a safe manner.
  • Recognizes safety hazards and initiates appropriate action to eliminate hazards and prevent recurrences.
  • Follows established infection control protocols.
  • If applicable, maintains data relative to Quarterly and Yearly Reports to the DOE/NYCDOHMH.
  • Work to provide comprehensive, personalized care for all New Yorkers and commit to providing a person-centered care environment while exemplifying ICARE (Integrity, Compassion, Accountability, Respect and Excellence) values.

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certification to practice as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

Qualifications For The Job

Two clinical years’ experience as Adult or Family Health Nurse Practitioner (NP) preferred.

Educational Level

I. Graduate of an accredited school of nursing.

2 Master's Degree in Nursing.

  • Licensed as a Nurse Practitioner in field of practice.
  • Licensed as a Registered Professional Nurse in the state of New York.
  • Excellent communication skills, both oral and written.

Knowledgeable In

Adult primary care services.

Equipment/Machines Operated

Spanish language fluency preferred but not required

Personal computer, telephone, fax, copier, Medical devices used in the course of training, i.e. glucometer, blood pressure equipment, etc.

COMPUTER PROGRAMS/SOFTWARE OPERATED

Epic EMR, Microsoft Outlook/ Office Suite

Years Of Experience

1 - 3 years' experience in an Ambulatory Care setting preferred but not required.
